Welcome, this is episode 525 of the Divine Intervention Podcasts. Into this podcast, we're going to be continuing the series on the 2024 Step 2 CK Free 120. This is going to be part 7. Again, I'll strongly encourage if you have the chance, listen to all the parts in this series, so much learning to get from this. So we've done the first 50 questions, let's go to question 51. A 13-year-old girl is brought to the office for evaluation of short stature. Since birth, she has been below the 30% alpha height and at a 30% alpha weight. She has never had a menstrual period. There is no family history of short stature, but her maternal uncle did not begin puberty until the age of 16 years. Her current height is consistent with that of a 9.5-year-old girl. Paul is 82 per minute and blood pressure is 110 over 70. Sexual maturity reading is stage 1 for breast development and stage 2 for pubic hair development, which of the following is the most appropriate next step in diagnosis. Option A says, complete blood count and serum chemistry panel. Option B says measurement of urine-free cortisol concentration. Option C says X-rays of the left hand and wrist. Option D says X-rays of the long bones and spine. Option E says X-ray of the skull. So if we look at this, this girl is short. And when we're dealing with short stature, many times people worry about two major kinds. There is either the familial short stature where your parents are short, it's kind of in the genes, so you're short.
And then there is a constitutional growth delay, where typically the child will be short, but the parents are not short. Typically, when you see that discrepancy, it makes you think that, you know what this child will probably catch up as purity draws to a close. Now, typically for you to make that judgment call on constitutional growth delay, for instance, familial short stature, you're usually going to compare the child's chronological age to the child's bone age. The chronological age is the real age. Their bone age is determined by radiography. And typically that radiography is going to be done with the hand and the wrist. So you then compare that because you can age bones on radiography. So you can see how old are those bones are. Typically, a child that has constitutional growth delay, which I suspect this girl has, their bones will be younger than their age. So that tells you that those bones have some catching up to do, and they will catch up. So this person should grow in the future. So the answer here is going to be C. When you want to assess short stature, these are one of those things where you either know it or you don't know it. You got to get the experience of the left hand and wrist. You know, shown B is wrong because that's what you're using to use to confirm. One of the ways you can confirm if a person has hypercord is all of them. Option A doesn't make any sense. There's no much is going to give you to help you determine why the person has a short stature.
All right, question 52. A 20 year old woman comes to the office because of a one year history of a lump on her left ear that appeared two months after she had the ear pierced. Physical examination shows the findings in the photograph, which of the following is the most appropriate initial treatment for this patient. So for looking at this image, this does look like a kiloid. So let's look at the answer to your option. E says cortical steroid injection option B says cryotherapy option C says laser ablation option D says needle aspiration option E says wide accession. So this person has a kiloid and remember, kiloids are pretty common actually. Excuse me, in African Americans. And one thing I would say is if you know that you have a kiloids acceptability, this could be easily be tested as a preventive medicine concept. A person that has kiloids sensitivity should generally try to not have their skin pierced or heavily manipulated because those things can lead them to form in a kiloids. So this question is clearly asking about the management of a kiloid. So how do we manage kiloids? Well, the first line treatment typically is going to be to inject steroids into the lesion. So the answer is going to be option A. Don't get me wrong. Can you also require therapy? You can. But honestly, cryotherapy is something that should be reserved more for second line.
First line treatment, although if you look at some studies, I believe some studies show that cryotherapy is super effective, probably a little more effective than steroids. Although many times I think if you combine both, you're actually going to get a better bang for your buck, but in general, you want to inject steroids into the lesions because remember steroids can inhibit fiber blast activity. So that would be very helpful in this case. So think of injecting is directly into the lesions as first line cryotherapy as second line. Don't do option E. Why the station is not helpful at all because the recurrence rate is almost 100% by trying to cut off a kiloid. And then option D neosyl aspiration does not fix the problem. An option C laser oblige and laser oblige is not as effective as options A or B. So I'm going to question 53. Of course, they put an image. So 42 year old woman is brought to the emergency department by ambulance 20 minutes after a head on motor vehicle collision in which was the restrained driver on arrival. She's alert and reports chest and abdominal pain medical history is on remarkable as she takes no medications. Temperatures 36.7 degrees Celsius that's 98.0 degrees Fahrenheit. Postes 120 per minute. So she's tacky cardiac respirations at 20 per minute. She's the picnic blood pressures 130 over 80. Postoxymatron foleters permitted of oxygen via nasal cannula shows an oxygen saturation of 90%. She is hypoxic. Breath sounds are decreased on the right.
3 Ks in the midline position. Cardiac examination discloses no formalities. An abrasion consistent in shape with a seat belt is present over the right upper abdominal quadrant. The area is tender to palpation radial and or salis pd. spouses and normal bilaterally. Results of lab studies are shown. So a ebg appeal to 65 millimeters of mercury that's low. A pco to 32 millimeters of mercury that's low. So she's hyperventilating which makes sense. So obviously a PhD 7.38 makes sense as well. I'm going to use 10.0 gram per deciliter. Chess texture is shown in addition to intravenous analgesic therapy which of the following is the most appropriate next step in management. So this person is a trauma patient and we see that this person is shot of breath. They're hypoxic and notice notice something here. This person has unilaterally decreased breath sound. What does that sound like? What does that sound like? I hope you're seeing divine. That sounds like a name of thorax. Again, notice I've not even said anything about the image. What if you look at this image? This is a pretty classic image for a new thorax. You may notice here that while on the side of the lung is white. It's that our longest block. Right? That block parts. It tells you that it's more like more x-rays able to go through it. Right? That's the part of the lungs that have colapse right because you're basically taking parincoma out of the way because the longest colapse.
So that's that's person has a has a new author X this person has a new author X of some sort. Well, how do we treat a new author X? We're going to place a chest tube. Of course, the USMLE is many times and I'm going to put chest tube as an answer. They're going to put the surrogate name. So let's look at the answer choices here. Option A says bronchoscopy. No, that's not the way you treat any more thorax. Option B says intubation and mechanical ventilation. Nope. Don't have to do that. All the time you're wasting doing intubation and mechanical ventilation could fix the patient. Option C says placement of a thoracic epidural. No, we don't do that. Option D says thorac and T says that's for plural of fusion. Option E says a thoroughcotomy. That's like making like a like a wide incision for the most part. The person's chest. No. Option F says tube for a cost to me. That is absolutely correct. That's another name for chest tube. Right. You put that chest tube. It's going to basically let that air out so the lungs can re expand. So F is correct. Option G says observation only. If a person has a small pneumothorax and they're chemodynamically stable, they're not hypoxic. Fine. You could do that. But this person doesn't need those criteria. This person is differently unstable. Definitely hypoxic. Intublice suggest tube. That's the two thoroughcotomy. Remember. A needle for a cost to me is the needle decompression. You do that when you're doing attention, your author.
The person's trachea is pretty midline. So this person doesn't have attention in the author. So attention in the author. You start off with a new decompression. Then after that you go to a chest tube for regular demothorax like this one. You're going to just do the chest tube. Another name for needle decompression as I've said is needle for a cost to me. Another name for chest tube is tube for a cost to me. The only time we should pick chest tube or tube for a cost to me is the answer for attention in the author. If you don't see an answer choice that says needle decompression. So I got a question 54. So 31 year old woman comes to the office because of a three month history of intermittent non-bloody diarrhea. The first episode began five days after a campaign trip in rural Virginia with her family. During the trip she drank from a spring-fed pond as she says was clean. During the past three weeks she also has had a five pound weight loss. That's 2.3 kilograms. Despite no change in appetite. She has not had fever or the gastrointestinal symptoms. She has no history of serocerosion as in dexinal medications. She's sexually active with one female partner. The patient is 163 centimeters. That's 5 for 4 inches tall and weighs 54 kilograms. That's 119 pounds. A BMI is 20 kilograms per meter squared. Temperature is 37.9 degrees Celsius. That's 100.2 degrees Fahrenheit. Pause is 85 per million in regular.
Rescriptions are 12 per million and blood pressure is 115 of our 70 millimeters of mercury. Abdominal examination shows mild diffuse tenderness to deep outpatient. Balsans are increased. Test of the stool for our cold blood is negative. Results of a stool smear are shown. Which of the following is the most likely effect in fetus agent? Option A says Campylobacteria Junai. Option B says Chostridioreides difficile. Option C says Antamiba Histolyrica. Option D says Eschericia coli. Option A says Gerardia Lamblea. Option F says Samonella Enteritis. Option G says Chicolitis Enteri. This question has diarrhea. When you see a diarrhea question on the USM, the first thing you always want to do is ask yourself. Is it bloody or non-bloody? Is it bloody or non-bloody? This is non-bloody. Right of the body, we can get rid of A. It's not C. It's a Junai that causes bloody diarrhea. We can get rid of F and G. Those both cause Samonella and Chicolitis both cause bloody diarrhea on the exam. We can also get rid of C. Antamiba Histolyrica. That also causes bloody diarrhea. Pretty much left with B, D and E. Okay, B, D and E, that's pretty mild and normal. So we just need to then start looking at what's in the question. Option B, C, D, we're going to see that we're going to see that we're sitting on an antibiotic exposure. That we don't see here. So B is gone. Remember, C, D, it can cause watery or bloody diarrhea on the USM, L, E is going to keep that in mind.
So now we're stuck between D and E, E coli and G, D. Well, if you notice, in this question, this person drunk from a pond. That's no smart. So typically it's a classic situation. You see with G. R. D. You see a person, they drink from a stream or a pond or whatever. And then they start having this non-bloody diarrhea that kind of lasts for a long time. If you see that, that's going to be G. R. D. All right, it's going to be G. R. D. If you don't be drinking from a pond, that doesn't seem like a very smart idea. So we're going to go with option E. Equal, I remember that is E. Take that causes travelers, diarrhea, this E. Take that causes H. US. So how are we going to treat this person's G. R. D. I. says, we're going to treat it with metronide dissolved. Metronide dissolved is very effective for treating G. R. D. All right. Notice there's this picture here. Of course, if you're going to recognize the picture fine, but honestly, I don't really care for it. If you just, again, I think I've said this many times in this series, if you just read your questions carefully, you're going to be pretty fine with these are, image questions. All right. Question 55 says, randomize placebo control clinical trial is conducted to assess the effectiveness of a selective estrogen receptor model later.
