Welcome, my name is Divine, this is episode 604 of the Divine Intervention Podcasts. And into this podcast we're going to be continuing the USM Lys Step 2 CK and Step 3 Rapid Review series. This is going to be series 123, series 123. Into this podcast I'm really going to be going over a lot of material that's actually really, really, really high yield to know for the USML exams. So, let's begin. What if they give you a question about an individual that just returned to the US from military, you know, thing like some kind of military deployment. And this was like, you know, the person just returned like four weeks, I mean, like three days ago. And this person for the past 48 hours has been having fevers and has been having a lot of you know, just fevers, shortness or breath. The person has been having a non productive cough. And the tell you that the person is brought to the emergency room and they give you some labs. You notice that the person's hemoglobin is like four. And they give you the hematocrate of like 12. Because remember your hematocrate is three x your hemoglobin. And then the person's white count is like 500 or something extremely low. And then the athlete leg count is like 3000 something extremely low. And then they give you vital signs. You notice the person's temperature is like 101 degrees Fahrenheit. The person is hypotensive. The person is tacky, cardiac persons to keep Nick. And then they tell you that they give you some relevant history.

They tell you that this person had an infection, a brain infection while they were deployed in some African country. And they received antibiotic therapy for that brain infection that they recovered from. And then they ask you, you know, what is the most likely diagnosis? And then our friends at the MBM is like I almost promise you that one of the answer choices is going to be malaria. They're going to put malaria as an answer. And you're going to put malaria as an answer. They're going to put drug toxicity as an answer. Just to kind of mess with your head, they can throw in something like, what's that thing from New England? That's that's with a... That can also cause him all the sense you know what I'm talking about. And it's not coming to mind now. When it comes to mind, I'll, I'll mention it. But that thing from New England that also causes like him all the sense, right? It's something that's carried as well. Yes, Babysiosis. There we go. Babysiosis. Right? Remember, it's also carried by the statistic. I know I have to think about that for a second. But Babysiosis, they put that as an answer. They can put like some kind of like malignancy as an answer, you know, just to kind of mess with your head. They'll just put a bunch of things, right? So of all these different options I've mentioned, which one should you pick? Well, I would really hope you're saying divine. I think the right answer is drug toxicity. And why is that? I know some of you may be like, man, divine.

This is a really, really bizarre presentation you're giving to us. Why is drug toxicity the answer? I think, before I talk about why drug toxicity is the answer, I think one thing that'll be one more useful is talking about why the other answers are wrong. So why would the other answers be wrong? Well, number one, malaria. Don't get me wrong. Can malaria cause fever? Yeah. Is this person a returning traveler? Yeah. Does this person have anemia? Yeah. But those malaria, those infection plus medium species cause panside opinea. It does not. Right. It doesn't cause you to have like a local pinia. It doesn't cause you to have a thrombus. Well, you may have a thrombus at opinea sometimes. But they're just certain facts of the case that are not really well explained. Right. And if you notice the trend of the fever I give you, I just tell you that it has fever. I don't tell you anything about like tertian fevers or or 10 fevers or anything like that. I literally see nothing about any of those things. So since I say literally nothing about those things, there's no reason why you should jump to those conclusions that, oh, yeah, this person has malaria. No, this person does not have malaria. But that be if I were an MBA me and I mean, that's what they'll totally do to you. Right. That's like a very reasonable answer to put on an exam. Right. And then Babysiosis is not an answer. Right. Because Babysia remember it affects red blood cells. Right.

So it can cause a person's red blood cells to explode. But and cause anemia. But again, Babysia, Microte does not really explain the person having the thrombus at opinea does not really explain the person having the leukopinea. Right. It doesn't really explain that. And by the way, remember, put a half malirium Babysia, they can have jaundice, they can have unconjugated hyperbular binemia because of the himolises that those box cause. And then don't remember, don't forget that Babysia is carried by the exodistic, the exodistic cause carries a Boralia Bocdofree. It carries an aplasma. And it also carries a Babysia Babysia Microte. You should definitely know that for your exams. Right. So those two are wrong. And then this is not a malignancy. Right. Because again, it just doesn't make sense. Right. Like malignancy, they will give you a little bit more than just panside opinea. They got to give you some clues. You know, the USM is they can just write questions where like this, there's nothing to guide you towards something specific. Although they actually begin to write a lot of questions these days where the questions are kind of non specific, but they somewhat expect somewhat specific answers from you. Those those kinds of questions require quite a bit of thought to navigate through. So this is not Babysia. It's not malaria. It's not malignancy. This is drug toxicity. Right. So this person took chloramphenicol. This person took chloramphenicol. Right.

Remember, chloramphenicol is a it's an antibiotic. It may not be used in the US for much of anything, but I can promise you that it's used in foreign countries. At least where I'm from, I certainly remember chloramphenicol being used quite frequently. I may or may not have been on chloramphenicol at some point in my life. Right. So chloramphenicol is certainly used. Right. And one thing that people forget with chloramphenicol is the chloramphenicol can cause a granulose itoses. Right. It can cause a granulose itoses. It can cause like a big time bone marrow suppression. Right. So that's why this person basically has a favorite on utropini actually from the chloramphenicol. Right. So again, watch out for a crime. I know many people think that all chloramphenicol can never be tested on my exam anymore. That couldn't be further from the truth. They may not test it in a person that's like US US US, but they will bring it with a returning service man or woman question. Remember, again, the US Emily is for the last couple of years, probably like two, three years, they've had this enhanced focus on returning service men and women. Right. That's something that everybody should know. So the if they're bringing a chloramphenicol question, they'll either make it in an immigrant or they'll make it in a military person that is just returning from deployment or a person that was like a missionary or a peace core volunteer. That's just returning. Right.

And got stuff, you know, antibiotic wise that you may not necessarily see in this country, but not necessarily seen the US. However, one thing I want to mention though is don't forget your other things slash drugs slash whatever's that can cause a granulose itoses and pancidopenia. Right. So for example, if a president takes these antithyroid drugs like pto methemazone, those can certainly cause a granulose itoses. If a president is clasping, remember clasping is a drug. It's like a third line drug for managing schizophrenia. So you should want to like start managing schizophrenia or clasping from the outside. That's not very wise. Right. You're going to have tried something for something second. If those two things don't work, they can go ahead and try clasping. Closipping can also cause a granulose itoses. Don't forget carbon mesopin. Carbon mesopin also has the ability to cause a granulose itoses. Right. So there's just many things they can test that can present as a granulose itoses. And honestly, friends at the MBA means for step two, step three, I think I said this. I say this many of my review classes. I say this to a many of my podcasts that see there is no one enhanced focus on basic sciences. Since step one became pass fail. The USM is almost like a way to get a handle on people still knowing their concepts from step one is to just create basic science questions. Right. So there's a lot of basic science questions you're going to see these days. It's just the truth.

Right. So the thing is they can literally write this exact same thing that I started with and then they'll say which of the following is the most likely theology of this patient's presenting symptoms. And then you'll put a bunch of mechanisms of action as answers. Right. So like for example, they can put like a fire peroxidase inhibitor. Right. Which is the mechanism of action of the anti thyroid drugs or they can put anti epileptic drug or whatever. Right. Which is the mechanism of action of cover me is a pin or they can pick they can put a D2 receptor blocker. You know, D2 receptor antagonist. That's how clasping works or they can pick poked fifth ribosomal inhibitor. They won't probably not put the 50s because they know that they put 50s. That kind of will joke some people's memories, you know, some memories from you know, some neurons or stuff firing from previous USM exam. So you may not see that on your exams, right. You may just put ribosomal subunits inhibitor or something like that. Right. So again, that that's the concept of a derivative. Right. You've heard me talk about derivatives so much, so much, so much, so much. Right. So the USM is the tick what you know and just translate it to a different language, right. It's like, hey, how can you confuse divine. Well, you know, divine, you know, first language is English, but hey, just speak a language that divine does not understand, right. You know, maybe I don't know.

So speaking like some language divine doesn't understand if you speak the language divine doesn't understand in divine just gets really, really confused fast. Right. So that's the same thing with the USM is right. That's why people say, oh, the exam was so weird and crazy and whatnot. No, it's not that the exam was really crazy or whatnot. They just put it in a language that you're not used to. It's the same concepts. They're still largely testing from year to year, although there's a lot of changes they've made over the last like, I'll say like 11 months or thereabouts, right.
