All right welcome my name is divine this is episode 519 of the Divine Intervention Podcast. In this podcast I'm going to be continuing the series on the step 2c3120. This is going to be part 2. So we do questions 1 to 5 in the first one. Let's go straight to question 6 and just plan through as many as we can. This will be a lot shorter than the previous one because I've kind of got in some thoughts out of the way. Okay so question number 6 it says. A 4 month old female infant is recovering in the hospital 5 days after uncomplicated surgical repair of a ventricle septal defect via medium sternotomy utilizing cardiopulmonary bypass. She was extubited on post-op D1 and by post-op D4 she was no longer receiving any anotropic infusions when travenous drugs. She's breastfeeding well, plans are made for discharge from the hospital the next morning. However on the next morning she develops a temperature of 38.8 degrees Celsius that's 101.8 degrees Fahrenheit, cool and clammy skin, cold distal extremities, delayed capillary refill and minimal urine output. Cardiac rhythm strip and tracing of arterial blood pressure are shown which of the fluency is the most appropriate next step in evaluation. Option E says cardiac catheterization, option B says CT angiography, option C says ecocardiography, option D says electrophysiology study, and option E says MRI of the heart. So here's something I'm going to see here.
So the EKG if you look at the rhythm strip, this person is pretty tacky-cordic, I mean their heart rate is pretty high, but it's a narrow complex tacky rhythm. It looks a lot like an SVT to me. Now I know some people will be sweating about like the arterial blood pressure waveform. I'm not going to go into that. Honestly whenever I see something that is non-classic in terms of a video or a graph or a diagram on the USML Es, I don't sweat it. I feel like a lot of people sweat it and they feel like wow, if I'm not able to analyze this complex looking diagram, I won't be able to answer this question correctly. You're just wasting time. Many times if you read a question carefully and you do just some simple deductive reasoning, you're being good shape. So for me at least an EKG, I feel like most people, I'm just trying to put myself in the shoes of a person taking the exam like a 30-year medical student or person just finishing 30. So this really should be shown on SVT. Okay, so for me, I then look at this and I'm like, okay, this person has heart cardiac surgery a few days ago, literally five days ago. And this person has now developed a fever, cool and clammic skin, cold distal extremities, delayed caprophil and all these problems. So I know that the heart cardiac surgery, I know that a lot of the question, a lot of effort was putting through the heart, the heart, the heart. So chances are I'm going to pick an answer choice that relates to the heart.
So option B, I'm going to cross that off right off the bat. I'm not going to be thinking of CT and geography. Option D, electrophysiology study is a good answer. Let's keep that for now. But option E, I'm definitely not picking. Again, I think I mentioned this in episode 518 that cardiac MR Is are almost always a bad idea on the USME Ls. They are one of those answers where they are various situations where they will ever be correct. And then option A says cardiac catheterization. Well, cardiac catheterization doesn't really make sense as an answer because I don't think we're worried about atherosclerotic disease in this patient. So I think that something we're going to skip. So we're kind of stuck between two answers. We have an echocardiogram or an electrophysiology study. Many times we're in situations like this, both of these relate to the heart and we can see that this person has an SVT, right? This person has an SVT. But the thing is again, so much of the question is not devoted to the SVT. So much of the question seems to be devoted to, this person's heart may not be pumping as well because we see this person has cool and clammy skin, coldness, the extremities, the lead capri field, they just had cardiac surgery. So many times on the exam, just speak the simple test. The simple test is going to be an echocardiogram. So I'm going to go to option C for this one. I'm going to go to option C for this one. I'll pick that over an electrophysiology study.
An echocardiogram is something that can be done very, very, very quickly. It's not something you have to spend too much time trying to accomplish. Okay, let's go to question number seven. So question number seven is classically, is a HPI question. One thing I usually tell people with this is when you see these kinds of questions just gloss through them. Look for the things that are very abnormal. Those things are going to stand out. These questions, they may seem scary, but they don't take too much time to read. So like for example, I'm obviously not going to read this whole question, but one good thing to focus on is, okay, there is an for the visit. My daughter's eyes and legs are swollen. Okay, that's very abnormal. And they read the HPI, you know, two weeks of cough, congestion running nose, parents report that the patient's eye seems swollen four days ago, which the initially attributed to the patient's cord. And then they have a two-day history of swelling over the tops of her feet. And this morning she was unable to put on the shoes because I feel we're too swollen, right? And again, like these HPI questions, just be a big picture thinker, right? This person, you have a kid with swollen legs. And this child had a URI for about two weeks. Okay, and then we kind of scroll through vaccines. I'm not going to stress about that. I look at the vitals. There's nothing that really stands out to me. I look at the physical exam. The skin, there's two plus feet in the DMA, right?
If you look at the HCINT exam, the person has parable on a DMA. And then again, the abdomen, there's mild tenderness to population in all quadrants. Right? You see a child, they just had a URI, they've been having a URI for like two weeks and see their legs are swollen. And then we look at the urine specific gravity, and we're told we see in the urine there's a lot of protein. So I hope you're thinking of nephrodic, some kind of nephrodic on ephrodic syndrome, right? Some kind of nephrodic on ephrodic syndrome. That's probably a smart thing to keep at the back of your mind with this one, probably some kind of nephrodic on ephrodic syndrome. So then the question is which of the following additional lab findings are most likely to be decreased in this patient, right? So again, since we're talking about nephrodic syndrome, we know that it's probably going to be something with albumin or complement proteins or whatever. Okay, so let's look at the answer. So it says hemoglobin, option B says serum albumin concentration, option C says serum C3 and C4 concentrations, option D says serum triglyceride concentration, option E says serum URI nitrogen concentration. So again, let's see what answers can we get rid of, right? Again, we're literally talking about nephrodic syndrome on an ephrodic syndrome or whatever, but hemoglobin, right? Hemoglobin has no relationship with nephrodic syndrome. We can get rid of that.
And if you think about it, when people have nephrodic syndrome, the liver actually responds in a very weird way. It's almost like the liver is like, hmm, I've lost a lot of albumin in my urine. I need to make up for a cardiac pressure in some way in the body. So many times the liver is going to respond by making a ton of triglyceride. So these people's triglycerides are going to be high. So D is out. Option E, if you think about it, if your kidneys are not working well, well, your GFR is going to decrease. If your GFR decreases, your creatinine is going to go up and your serum URI nitrogen is probably going to go up as well. So that's going to be wrong, right? So we're now stuck between serum Abiumin concentration and serum C3 C4 concentrations. So how do we tease between these two? Well, the thing is, when you see low complement proteins many times on the USML exams, those are usually going to be given in the setting of post-traffitococcal glomerulonephritis or in a person that has a membrane or proliferative glomerulonephritis. But the thing is usually those two things present as nephritic syndrome. Many times they present as nephritic syndrome. Many times they present as nephritic syndrome. So I'm like, hmm, serum C3 C4, right? And then B says serum Abiumin concentration. So I kind of stuck between these two.
So whenever you're in these kinds of situations, it's just always helpful to ask yourself, which one do you think will pretty much always be right, regardless of what's going on? Like, for example, the serum C3 C4 being low is something we see in two very specific pathologies. The thing is, do we know the specific pathology that this child has? No. But we know that this child has the ephritic syndrome or nephritic syndrome. But option B, low serum Abiumin concentration will that be right in pretty much every situation? Yes. So because it's almost like that will cover your basis all the time. Option B makes the most sense as an answer. So good option B, low serum Abiumin concentration because the PM, a lot of protein, if that happens, that's going to lower the oncotic pressure of your bloodstream. And if you lower the oncotic pressure, you're going to have more fluidic extrafacation. You're going to keep fluid in your vascular tree. So you're going to get in some trouble. All right. So this is good question number 8. Question number 8 says, a nine year old girl is brought to the clinic for a routine examination. She has a four year history of asthma. Her only medication is inhaled out of her own with a spacer as needed. She uses it only occasionally and has not used it at all during the past four weeks. Her father reports that his daughter has an episode of coughing that awakens her at night to once a week.
He also says that his daughter has a sedentary lifestyle because she coughs if she runs too much. No one in the family smokes cigarettes and there are no pets. Gas heaters are used in the home. The patient is at the 25th percentile for height and 90th percentile for a weed and BMI. Respirations are 16 per minute. Longs are clear to all scotation. There is no clubbing of the digits. The remainder of the examination shows no abnormalities, which of the following is the most appropriate next step in management. So option A says add oil theophilate to the regimen. Option B says add flutica zone by middored dosing healer with a spacer to the regimen. Option C says add some mineral by middored dosing healer with a spacer to the regimen. Option D says, begin a 5-day course of oral pregnancy. Option E says no change in management is necessary. So it makes obvious sense this child has asthma. Pretty straightforward. This child has asthma. You know we're told that she uses inhaler all and only use occasionally as many as during the past four weeks. Then it seems like she has symptoms about once a week. Right? So basically this question is asking about asthma management. So what are some answers we can get rid of right off the bat and then I'll discuss the concepts. Option E makes no sense. Right? Given the ophiline. Basically the ophiline is one of those dead-on-arrival answers. It's all it is just highly unlikely that the ophiline will ever be.
And I want you all to encourage you as I'm going through this podcast or this series. Make a list of these dead-on-arrival answers. The ophiline. I just cannot see a situation where it will pretty much ever be correct. Would it be correct? Yes. But again very rare. The ophiline is like an endangered species on the USML Es these days. And then let's consider BCD&E. The thing is many times with asthma management, one thing that just gets many people is they're like, oh divine.
