Okay, welcome. My name is Devine. I am a PGOI, no, no, PGOI. PGOI to redeal your resident. So I guess I'm an R1 resident. And in today's podcast, I'll be continuing our rapid review series for the US Family Step 2 CK exam. This is going to be more of kind of like random, but it'll be a rapid review for Step 2 CK. Okay, although we focus a lot on like medicine topics. So let's go ahead and begin. So what if you get a question about a patient that is, you know, hypotensive, you have a temperature of 103, they have crackles that your school takes like at the left long base, right? And if you had a productive cough for like two or three days, what am I going after? That's a pneumonia, right? That's a pneumonia. Well, let's assume this person is coming in there like hypotensive, the attackic heartic, their febrile. What state is this person in? The person is septic, right? The person is a state of sepsis. So for person is in a state of sepsis, what kinds of bugs would you like to cover? You would want to cover mersun su domonas, right? For the most part. I mean like in the real world, like you will try to be brought spec in the real world, but there are certain kinds of sepsis where like if a person is like you has like euro sepsis for example, right? Like sepsis from a UTI, you probably don't need to cover mersun or usually doesn't make much sense to do that. But again, it just sort of kind of depends on, but this is MBME work. If a person is septic, right?

Like non-specific and usually if a person is septic on MBM is the one give you a specified source of infection. So if the person is like has like a septic picture, what are the two bugs you want to cover? Mersun su domonas, right? So what are the drugs that cover mersa? So those will be drugs like vancomycin, right? Remember vancomycin, binds to the ala, right? So that essentially prevents the formation of the bacterial cell wall. Remember there's lignizolid, the nasolid is a 50s inhibitor, it's bacterostatic. Remember that thing can trigger serotonin syndrome. Remember daptomycin. Remember daptomycin only covers gram positives, right? It basically like pukes holes in the walls of a, in the cell membranes of a bacteria. And then remember your septaroline. Remember septaroline is a fifth generation cephalosporine, has the ability to cover mersa. Remember bactrin, trimethoprimsulfomethoxazole, also covers mersa. Remember clindamycin. Clindamycin is a 50s inhibitor. It also does cover mersa. And then don't forget there's one other drug, doxycycline. Doxycycline does actually have the ability to cover mersa. Okay? Now, how about pseudomonas? What are the drugs that cover pseudomonas? Well, I hope you're thinking about septazidine. Remember it's a third generation cephalosporine. Remember it's the only very high yield. Most, essentially third generation cephalosporines do not cover pseudomonas with the exception of septazidine. Okay? How about cephepine?

Cephepine is another cephalosporines of fourth generation cephalosporine, has the ability to cover pseudomonas. Remember as trionam, it's a monobactam, also covers pseudomonas. Remember your fluoroquine alums like moxifloxacin, gadifloxacin, livofloxacin, ciprofloxacin. Those all cover pseudomonas. And then don't forget your amino glycosides, right? So like genetomycin, neomycin, amicacin, tubromycin, streptomycin. So your protein synthesis inhibitor is, remember it's bacterial cyno, very good for covering pseudomonas. And then remember that you have amino glycosides, right? You do not cover an aerobus. That's something high yield to know on MDME Excepts. And then don't forget that your paparacelin-thizobactam has the ability to cover pseudomonas. And then also do not forget that there's one more drug class I'm missing out on here. I guess I've covered the big ones. Yeah. But those are the drugs in general that cover pseudomonas. Your meropenem, right? So your cover penems, like meropenem, iripenem, meropenem, imipenem, buri penem, buri penem, erdi penem, those drugs all cover pseudomonas. Okay. Now what if you get a question about a person that over the last, like, you say over the last two months, they've been feeling tired, they've just been feeling like not well. And then they give you labs and you notice that this person is hyponychromic and this person is hypercholimic. And then they show you like a CBCI notice, that this person's your synophils are innovative.

What are we thinking about? And let's assume this person's blood pressure is like 80 over 50. What does this person have? So again, sodium is like, I don't know, like, 180. And then potassium is like 5.9. And then the I use synophilic count is like 15%. What do these people have? They have adicence disease, right? They have adicence disease. Remember adicence disease, you have like an autoimmune destruction of your general cortex, right? And remember, if you have one autoimmune disease, that increases your risk of having another autoimmune disease, right? So if a person has like Hashimoto's, they can have adicence, or a person has adicence, they can have Hashimoto's, right? So kind of like that. And they can also have other things like premature variant failure from an autoimmune destruction of their ovaries. They can have pernisiosanemia from making antibodies against their parietal cells. They can have previous disease, right? So they can have my astenia gravies. They can have all those problems, right? So remembering adicence disease, which is the main subject of this question, you make autoantibodies against your general cortex. So look out what you knock out, right? You knock out your zonal glomerulosa, which makes mineralocorticoids like out dostron. You knock out your zonal fasciculata, which makes glucocorticoids like cortisol. And then you knock out your zonal reticularis, which makes, which makes sex steroids, right? Like DHE, yes, an angiosteen dion, for example.

So the thing is, if a person has primary genocency, or adicence disease, they don't have the effects of our dostron anymore. So because they don't have the effects of our dostron, remember our dostron? If you have high levels of our dostron, say, for example, in con syndrome, you'll have a high perni-tremia, and you'll have a high pochilemia. Because if you remember the principal cell of the collecting duct, right? Under the effects of our dostron through like the inek channel, you reabsorb sodium, you don't potassium in the in the urine. So if you have an outdostron deficiency, right, from adicence disease, you'll be high per, you'll be high pochilimic, because you're no longer reabsorbing sodium, and you'll be high perkylimic, because you're no longer dumping potassium. And then, in addition to that, if a person has primary adicence efficiency, or adi-haven, you're xenophilia, right? So you may see, okay, divine. Why does that make any sense? Because if you actually remember like the numonic, the differential diagnosis for you're xenophilia, adicence disease is one of them, right? So remember again, if you have adicence disease, your zona fasciculator is all gone, the numicine cortisol, the thing is glucocorticoids actually cause hip-optosis of lymphocytes. So if you have less in the way of cortisol, you have less hip-optosis of lymphocytes. So if you have less of lymphocytes, I mean, sorry, what am I saying here? Your glucocorticoids cause hip-optosis of eucenophiles.

I apologize for that, because the poptosis of eucenophiles. So if a person has less cortisol, they will have less hip-optosis of eucenophiles, so their eucenophil count to a rise, and they'll have adicence disease. I mean, they will have a leusonophilia that is characteristic of adicence disease. And I mean, if they want that to integrate this on an Mbim exam, if a person has adicence disease, they will very likely have like a metabolic acidosis. Again, because remember that alpha-intercalidate cell that you find in the distone nephron, that alpha-intercalidate cell pumps out protons into the urine. So if that alpha-intercalidate, if you don't have our nostril, you're not pumping out those protons. If you're not pumping out those protons, you retain hydrogen ions, so you get a metabolic acidosis. If you want to be a little more specific, you will get a non-anion gap, metabolic acidosis. If you want to be a little more super-specific than that, because you're in a hypo-auto-steroan state, those people will have type 4, or like a collecting tubal-based renal tubular acidosis. Obviously, if a person has adicence disease, one provocative test you can do is to give them ACTH. I think there is this analog of ACTH, if I'm not mistaken, that's known as Cossin Tropin. You give those people ACTH. Their cortisol levels will not rise. So failure of a person's cortisol to rise with the administration of ACTH, or an ACTH analog, is diagnostic for adicence disease.

And obviously, if a person has adicence disease or primary adrenaline sufficiency, you'd want to treat them with like flusual cortisol. Flusual cortisol will help them replace the aldosterone, and then you also give them like prednisone to help them replace that glucocorticoid effect. Now, if a person has secondary adrenaline sufficiency, will they have hypo-calemia and hypo-neatremia? I hope you're saying no, because remember, if for example you have an issue at the level of the anterior pituitary gland and you're not making ACTH, if you're not making that ACTH fine, whatever, right? You won't make cortisol, excuse me, you won't make cortisol, sorry, I'm not feeling too well today. So you won't make cortisol, and you won't make sex steroids, right? So if you're not making any of those things fine, but remember that our dosturone is not under the control of your ACTH system. Our dosturone is under the control of your reigning and your reigning and your dancing too basically has way can do its job. So that's kind of like a high-yield thing to know.
